Prepare for your Office Visit

In addition to completing the New Patient Packet, we will also require you to bring the following information:

    • Driver’s License or another valid government-issued photo ID
    • Insurance Card and any co-payment due
    • If applicable, your physician referral form (you can request your physician fax, email or complete our referral form online to save you time)
    • Any X-Rays, Lab Work, EMG, etc.
    • List of any medications you are currently using
    • List of medications used in the past for your current concerns
    Diagnostic testing performed in the past including MRI, CT scans, EMG/NCS testing, current lab work

If you would like to be evaluated following an accident or injury, please call our office at 703-778-1800 to receive a secure link to fill out the form in addition to bringing your attorney contact information.
